I appreciate that Coaching/Hypnosis/Guided Meditation/Counselling is a protocol of helping the body heal itself through natural means.

I understand that Emily Wysock-Wright does not diagnose medical conditions.

This programme is not a substitute for professional medical treatment, although it can in some cases help manage medical conditions.

Emily's Duties:

I shall provide the services with due care, skill, and ability. Due to the nature of coaching, I do not guarantee any particular results. If for any reason I am unable to provide the services at the agreed time, for example due to ill health, I will provide you with as much notice as possible and I shall offer an extension or your coaching services as appropriate.

Confidential information:

I acknowledge that in the course of providing the Services I will have access to Confidential Information relating to you and your affairs and I agree not to (except in the proper course of my duties) use or disclose to any third party any Confidential Information. This restriction does not apply to:

- any use or disclosure authorised by you or required by law;

- any use or disclosure which I in my absolute discretion consider necessary or advisable in order to prevent illegal acts or harm to you or to others; or

- any information which is already in, or comes into, the public domain otherwise than through my unauthorised disclosure.

Fees:

In consideration of me providing the Services to you, you will pay me the fee agreed on completion of joining the programme.

The fee is payable on receipt of accepting your proposed package. If payment plans have been arranged, further payments for the programme will be automatically charged to the card used at booking for the duration of the plan. This is a financial agreement.

Without prejudice to any other right or remedy that I may have, if you fail to pay me on the due date, I may:

- Charge interest on such sum from the due date for payment at the annual rate.

- Suspend all Services until payment has been made in full.

Terms of Engagement - Myself and The Client

No claim can be made about the efficacy of this programme.

The Coach:

- Is not permitted to diagnose or claim to treat medical conditions. Coaching is not a substitute for professional medical advice and treatment.

- Maintains high standards of professional and personal conduct.

- Refers client to a general practitioner or another practitioner where necessary.

- Respects the confidentiality of the client by using patient codes. Personal details are kept by the practitioner; in accordance with the code of conduct outlined by the Information Commissioner’s Office. Information can be passed onto another practitioner with your written consent. Any information that breaches Public Interest will be disclosed. Freedom of Information Act 2000.

The client:

- You are responsible for contacting your GP or specialist about any health concerns you may have.

- Please advise your GP of the nutrition protocol you will be following.

- Please also advise any other complementary medicine practitioners you are consulting.

- It is important that you tell your practitioner about any medical diagnosis you have received any prescription medication, herbal medicine or food supplements or over-the-counter medication you are taking as it may affect the nutritional programme.

If you are unclear about any part of your plan and the programme, then you should contact Emily immediately for clarification.
